1033 State Decision Repository Summary Report

The 1033 SDR Summary Report will generate a list of entities with one or more common SDR activity elements, such as the same activity code or the activity within the same state.

  1. Click the Summary Reports link located under the Tools tab.

  2. Select the 1033 State Decision Repository summary Report option within the Market Regulation area of the page.

  3. Read and Agree to the Terms and Conditions. The Search Criteria page will display.

  4. Enter the selection criteria. At least one field must be completed. Selection criteria for this report are as follows:

  • Entity Role
  • Activity Codes

    • 1033 Written Consent Approved (APR)
    1033 Written Consent Denied (DEN)

  • Date Effective/Filed (From/To)
  • Date Entered (From/To)
  • State of Activity
  • Report Options

5. Click the Submit button. The 1033 State Decision Repository Report page will display.
