Company Appointment Renewals
The Company Appointment/Termination Report application
allows you to view, print, or download as a file a report of the active
appointments, the appointment terminations, or appointment/termination history
for a specific company in a specific state.
There will only be one company and one state per report.
The
report contains a summary of active appointments and/or terminations (depending
on search criteria entered) for a specific company, and the following company
details:
- Name - company name
as state regulators submitted to the NAIC
- NAIC Short Company Name - standardized company name
- NAIC Company Code
- FEIN -
Federal Employer's Identification Number
- Business address
To access Company Appointment/Termination Report:
- Click the Utilities link under the Tools tab.
- To access Company Appointment/Termination Report, click the Company Appointment/Termination Report link in the PDB Utilities section on the left side of the table toward the bottom of the page. The Company Appointment/Termination Report page will display.
To create a report:
- Click on the Create a New Company Appointment/Termination Report button and then click on the Submit button.
- Enter either the FEIN or Cocode of the company in the appropriate box then click on the Submit button.
- Select the State or States by checking the box(s) or check the All States box to get a report on each State. Each State selected will generate its own report.(Note: Only the states that the company is licensed in will be available to be selected)
- Select the type of report: Active Appointments, Terminations, or Appointment/Termination History.
- Select the Producer Type: Individuals Only, Agencies Only, or All Producers.
- Click on the Generate Report button.
- The Successful Submission screen will display your report selection criteria.
- Click on View status of submitted reports link
- A list of existing reports page will show all of your current batch reports and their status.
- A ‘Pending’ status indicates a report has not yet been generated.
- A ‘Complete’ status indicates a report has been generated and is available for viewing, print, and/or download.
- You are also given the option of generating another batch report.
- Clicking the Refresh button on your browser’s tool bar will update the status of the report.
- When Complete the report can be Browsed, Printed, Downloaded or Deleted.
- The Print Report allows the user to print a large report in sections. If there are more sections to print, the “Next Section” link will be available at the bottom of each section. If the last column of the report cuts off when printing, go to Page Setup and reduce the left and right margins to a smaller size.
To Download a report:
- Click the Download Report link from the Report Status page once the report has successfully batched.
- The File Download dialog box will request that you save the report. Click the OK button.
- The Save As dialog box will request a location of the saved file. Once this is given, click the Save button.
- The Downloaded file should be saved in comma
delimited (CSV) file format.
